Please try to reload the page. If reloading the page does not work, the issue might come from an ad blocker you might be using in your browser, which can (accidentally) block our second questionnaire from displaying correctly.

We suggest that you either try using a different web browser or try disabling the ad-blocker on your current web browser (or sometimes referred to as an “external content blocker”). Then try logging in to the portal again.


Alternatively, if this doesn’t work, we advise that you clear your browsing history and cookies (please log out of the portal first). To clear your history and cookies, go to Settings and clear Cookies and Website Data. Then log back in as normal.
